To make an appointment, it is best to contact us by phone (leave a voice message if necessary) or e-mail. You can also come by and make an appointment directly at the reception desk.
If you are not able to come to the scheduled appointment, please let us know at least 24 hours in advance.
Please remember to bring your insurance card to the appointment.
